In February, three alarming “never events” occurred within hospitals governed by the Betsi Cadwaladr health board, casting a spotlight on severe patient safety lapses that could have been prevented. Surgical Errors Expose Patient Safety Risks
The first incident involved a new mother who had undergone a Caesarean section but was mistakenly fitted with a contraceptive coil—a procedure neither intended nor required. In another case, a patient narrowly avoided an incorrect surgery that targeted the wrong toe, underscoring the potential repercussions of procedural errors. Furthermore, a patient with swallowing difficulties was mistakenly administered oral medication intravenously, posing grave health risks.
Medical Negligence Raises Concerns
Alarmingly, the Betsi Cadwaladr health board was responsible for a significant proportion of never events in Wales, accounting for 12 out of the 37 incidents reported between April 2021 and March 2022. Such disproportionate figures raise concerns about systemic deficiencies and call for an exhaustive review of existing safety measures.
