How Do Our Case Managers Work?
Care Assessment and Planning
Once allocated, a Case Manager will assess the health and social needs of the client and put in motion the necessary help for the client live a full life, ranging from appropriate accommodation, educational support, ongoing therapies or clinical treatments.
A Collaborative Process
The client of a Case Manager is at the center of multiple spheres of influence that will impact and shape their lives following their trauma. The Case Manager will work with support services, medical services and social agencies as well as those funding the individual’s care to ensure they are supported in the way their condition requires. In doing so, cost-effectiveness for all parties is ensured and the client will receive the best possible chance of a successful rehabilitation.
Financial Liaison
Our Case Manager will be the point of contact with the legal or insurance fee-payer, ensuring the client’s position is advocated for at each stage of the litigation process and funds are used appropriately.
Recruitment and Employment
Where necessary, our Case Managers will research, recruit, train and supervise the support teams or professionals needed for the ongoing care of the client. This can mean directly employing people or instructing appropriate HR agencies.
Living Adaptations and Equipment
We work with statutory services and monitor funding as well as with private therapists and services to ensure all appropriate living adaptations and equipment are in place for the client.
Educational Needs
Our Case Managers can oversee the writing of Education Health Care Plans (ECHP) for those with special educational needs, whether children or young adults.
Promoting Client Independence
The Case Manager will advance the client’s independence and ability to fully engage with their community whether for voluntary work, leisure or employment.
